I was told we would be having a meeting regarding my moms return home on specific day. That didn’t happen and After 5 days still hadn’t received any calls back. I had to go to facility for a visit to actually speak to someone. On the day she was leaving the facility i had to wait outside an extra hour to pick her up because the paperwork wasn’t completed by them. But the topper was when we got home we realized they sent another residents medical paper work with us which is a HIPPA violation.I’m absolutely disgusted with this facility.

I would give them zero stars if I could. My Mom was sent there after there was nothing else they could do for her so for end of life care. I flew up from AZ to see her - This was in January 2021 - checked with them and they said no problem. I got there and they refused letting me see and say goodbye to my own Mom! I asked what can I do and the gal on a power trip told me I could get a Covid rapid test. She had me wait while she got the test site info for me and came back with her coat and purse heading home and said she changed her mind and said I needed to wait 10 days. My Mom didnt have 10 days and I didnt get to say goodbye. I will NEVER forget or forgive what they did for no good reason. And no they werent just being careful!

My Mom was admitted to Montesano Rehab and Health Center this last May after suffering a debilitating stroke. She went from being a fully independent and lively 87 year old to a long term resident in need of full time care. My family and I are so grateful that this skilled nursing facility had a bed available, that this is her new home. Stacie in admissions was so compassionate, just took charge of making my Moms comfort and safety a priority- settled her first in her new room then worked with me on admitting paperwork. Stacie had been working with us for over two weeks to coordinate this admission ( a transfer from hospital, etc)and had all the papers ready so it was quick and painless. The entire office staff, the faces that greet you when you arrive, are a wonderful group. Belinda, Renee, Caron, Judy- thank you for your kindness and hard work. Not easy navigating visitors schedules during a pandemic but they do it with grace and a good sense of humor! A huge thank you to the nursing and therapy staff- Annette, Director of Nursing, Stacy, Jennifer, Mikayla, Lily, Shirley, Carrie. I wish I could list all of you, it takes a village to care for these residents and due to limited visiting hours it’s impossible to meet all of you. Especially the night time staff of guardian angels. Thank you! For those of you like me, looking for the best place for your loved one- this is it. Montesano Rehab Center is located in a beautiful setting, quiet residential neighborhood with lovely courtyard for residents and visitors to enjoy. Exceptionally clean facility, nicely decorated in a non- sterile decor, much more homey than you’d expect. The kitchen staff does wonders, my Mom always has her favorite foods to tempt her which is very much appreciated. Lemon cake for lunch, no problem! My family and I only have one area that we feel could use improvement and that would be communication between staff and the residents family. We understand that staff is busy and we know what great care they are giving our loved one but to be kept apprised without having to ask- that would be so helpful! Maybe in the future an online patient portal where we could read notes from Doctors, nursing staff- updates on therapy, meds? Or a designated staff member who reaches out with updates on a weekly basis?
All in all… hands down best place on the Harbor for you or your loved one!

My mom had her toe amputated and she was sent there to recover. Got a call that her wound was leaking but they had an order from the doctor to not change the bandage. They reassured us it would be taken care of at her Dr's appointment the next day. Got a text saying her appointment was canceled and they would reschedule for the next which I never heard anything back. They left her leaking wound sit there and didn't change or even look at her bandages for at least 9 days and now they're going to to have to amputate her leg. And since she contracted covid19 while being there, there's a chance she might not make it. They wouldn't tell us what was going on with her. They said we needed power of attorney for them to tell us anything. The director started there weeks ago, the same time my mom was admitted there. If you want to go down hill quickly, get covid, and die due to poor treatment by all means go there.

So my mom just tested positive for Covid earlier today, but no staff member has notified me. I found out because I was on the phone with my mom when the nurse informed her. Im her POA and family. I would think their 1st priority would be is to inform family members of this illness, not the next day or when they get around to it. Since it could be a life and death situation they should be notifying family members right away.

I wish I could give a zero star review. My girlfriend worked there at the end of December and the other workers there are disrespectful and unprofessional. Dreama failed to put that my girlfriend worked 5 days and instead put 2 days. We solved that issue last week but she still has failed to send my girlfriend check in over 2 months. When we called she yelled at us for calling. So unprofessional

Terrible place. There is a reason these places are often referred to as "don't care facilities". This is it' They often don't listen to important needs of the resident. They tell you the rules and then change them to suite their needs not the the resident. The staff seems very unhappy. Hearing them complain amongst themselves especially late at night makes it even clearer.
