My mother was in Cibolo Creek as a long term resident. She was there for over 10 years and I cannot say enough about the care my mom received. The staff was very sweet and concerned for my mom. I was always kept up to date on what treatments she had what medications she took. Unlike some reviews I read I did not smell urine all the time. Sure if someone was being changed and I happened to walk by. But I rarely smelled anything. That stuff happens anywhere. My mother’s room was always clean my mom was always clean her hair was washed and brushed. Linda Lee the activity director was exceptional in keeping up with the activities for the residents. They were always doing something. She was great in getting my mom up and out to play chicken foot dominoes, music, bingo and holiday activities. Physical therapy was great too helping to get my mom in and out of a car when I took her out. The nurses were awesome in her care as well. Always communicating with me about her care since I lived in Florida. I had considered moving my mother out to another facility near me in Florida and decided against it because I felt her care at Cibolo Creek was the best I had found. I have a friend who would drop in for a visits and the facility was always clean as well. I will be forever grateful to the Cibolo staff for taking such great care of my mom.

Exceptional Rehab Care at Cibolo Creek Health & Rehab in Boerne

Cibolo Creek Health & Rehab is the best rehabilitation and long-term care facility in Boerne. My grandfather spent his final years here, and the care he received truly transformed his quality of life. Thanks to the dedicated staff, he regained strength and independence, allowing him to enjoy a fulfilling social life that wasn’t possible at home.

The team at Cibolo Creek is incredibly compassionate and attentive, even when working long hours. Their commitment to patient care is evident in every interaction—they always prioritize residents’ well-being, no matter the circumstances.

If you’re looking for a top-tier rehab and skilled nursing facility in Boerne, I highly recommend Cibolo Creek Health & Rehab. Our experience was life-changing, and I hope others can benefit from the same exceptional care.

My journey with Cibolo Creek started in April 2021 when my Dad fell and broke his foot. It was during Covid so restrictions were everywhere.

After Dad's surgury (age 95) he was told no weight on that foot for 10 weeks! I was devastated about having to leave him somewhere. I called The Center in Boerne and spoke to the director, Olivia Burdick. She immediately said Cibolo Creek is where she would strongly recommend.

I thought he would move back home with me after the 10 weeks but he said he liked it there and he felt it was the better option. He said if he came home with me I would need help to care for him and he was very well taken care of there. I was able to visit him daily and he had found his new home.

He loved a schedule so he went down to the rehabilitation gym every morning after breakfast and they helped him get set up on the bicycle...even if he wasn't on PT. That's the way they roll!

The activities director, Linda Lee was challenged with Covid restrictions but she did everything allowed to let the residents socialize. She had live music several times a week. Dominos was started due to a request from Dad. They played bingo and he was even on the resident council. It was his home and he enjoyed his time there. He passed in January of 2023 and my husband and I were there with him. He had just had a bad morning so they called me to give me a heads up.

The nurses and every member of the staff treated him and us as family. At the time of my Dad's passing my Mother in law was also a resident at Cibolo Creek. She was also well cared for and enjoyed the extra attention.

My Mother In Law and Father In Law have both been patients there over the past 4 years. My Mother In Law is currently a resident there for long term care. Her husband passed away there in August and they got her set up with grief counciling and took extra time with her to help her adjust to life without him.

We even had my Father In Law's celebration on life there in the dining room in August 2024. The staff moved all of the tables around and had it set up with table cloths and helped make it all run smoothly.

I've recommended Cibolo Creek to numerous friends for their loved ones and all have been very pleased with the care they received.

I've never been in there when the place wasn't clean. I never smelled anything and I know that's rare for a nursing home but they're on top of it.

Marjorie, Taylor, Karen, Linda Lee, Keela.....the list goes on and on with the staff that have moved mountains to make life better for our loved ones.

I'll continue to highly recommend Cibolo Creek Health and Rehabilitation to anyone in need of their services.

My sister is a resident at Cibolo Creek Health and Rehabilitation Center. The staff is friendly, considerate and professional. They are very supportive and patient with her and they encourage her to get involved in the daily activities that are offered.
I am glad she is at a facility that truly cares about her physical and emotional needs.

My sister was there for rehab after falling and breaking her hip and elbow. The place is a disgrace. We visited nearly everyday and everyday she declined more. The place smelled strongly of urine. I told them that the smell of urine in the halls was nauseating. They told me that diapers are changed, but not always taken from the room. The bathrooms were filthy. We were required to wear a name tag, however very few staff had one, including the administrator. I would request that my sister have a shower and they lied and told me she got one every other day. I made sure she had a shower bag with all the personal hygiene items she would need. I had to wash her face everytime I came and worked to get the dry crusties out of her eyes. I also had to wash her hands(she had dirt under her fingernails) and brush her teeth and hair. We made sure she had clothes, but they would dress her with clothing from the lost and found. When I opened her drawers and closets to show them her clothes, the nurse told me she didn’t want to go through her personal items. When visiting I would find her sitting in a wheelchair with her head back and her mouth wide open. She was overly drugged and unresponsive. I needed to ask for a pillow behind her head. Her lips were dry and stuck together. She was thirsty and hungry. She smelled like the smell of the dirty facility. I saw people in the lunch room crying for help with their meal. When no one answered their calls my husband went and helped. Therapy (PT) consisted of her sitting in a wheelchair while someone sat by her. I told them that at the hospital she was assisted in getting out of bed and walking four feet, turning around and going back four feet. They continued to use a hoist and never even helped her sit up. On the last day she was there my nephew found her laying in bed, overly drugged, food particles in her mouth, dirt in her fingernails, hair not combed and stuck to her head, and foul smelling. He was very upset, told them he was taking her immediately. He literally lifted her from the chair and put her in his truck. He took her to Mary’s House where day by day she is getting better. We were disgusted with the shape she was in and how unclean she was.
Shame on this place!!! I never knew we cold treat our vulnerable so poorly. Get your loved ones out of there and find a place that cares. I’m not stopping until I am heard.

After a few months, contemplated posting a review however I feel it’s necessary.
My grandma was placed here last year for rehab and the first time was wonderful. She went home and had to come back for rehab a few months later. In that time she had shingles, Covid, and was not eating however we were unaware & staff would say she was eating when we would visit. We couldn’t get through to the ADON/ DON after multiples attempts. Before moving her to another facility I visited her and she was (without too much info) not doing well, I talked to the nurse and asked her to please call the doctor, to which the nurse stated she was just acting like that for us, and that she was eating, and doing well during the day. I again asked her to call the doctor to which there was no rush and continued to snack on her chips while talking to me. That night we took my grandma to the ER on our own and she had major infections going on. We were in the process of moving her to another facility the following day, so we brought her back for the night. The next day my grandma passed away. We were not notified for 2 hours after she had passed away. We let the nurses know we were on the way however they already had called the funeral home to pick her up stating it had been hours (however it was 25 minutes between the phone call and us arriving). The nurses do not care, the management team does not care, heartless. I wish I could save everyone from putting their loved ones here however I know that’s not possible. Just please reconsider.
