Do NOT send your loved ones here if you value their health and well-being. The images on their Google profile are blatantly misleading and do not reflect the facility’s actual condition, which is shockingly filthy, neglected, and run-down. I can’t imagine where they got those pictures, as the reality is truly appalling. The entire place reeks of urine and waste, the rooms are dirty, and staff routinely ignore residents’ pleas to contact family members.

My mother’s health rapidly declined under their so-called ‘care,’ culminating in a severe blood infection caused by an untreated UTI and E. coli. The infection became so advanced that she was left unresponsive, unable to move or speak. If we hadn’t intervened in time, she would not have survived another day. They not only failed to provide basic medical attention but also seem to overmedicate residents to keep them subdued, sparing themselves the effort of proper care.

Please, if you’re considering this facility, take the time to visit and witness the lack of cleanliness, compassion, and basic human decency firsthand. This place should not be allowed to operate. For the safety and dignity of your loved one, stay far away—they deserve so much better.

My relative had a good experience at Country Hills. They received the therapy they need to be able to return home. We appreciate the help we received from all the staff.

Some of the Cna's don't want to want to do their job it took 5 days after being here to see a doctor for any kind of meds & pain meds.... now I do have to say that I loved & enjoyed PR/OT the people are super nice ....then there's Damien the kitchen manager, now that guy is super duper sweet and nice & makes sure they get your food right, but when he's not around the food is inedible & put food on your tray that your allergic to, thank goodness for door dash/Walmart (lol) the people in admissions and social services are all really nice and get you the things you need.

Overall I really didn't like being here, but some of the people made it tolerable.....

They are truthful, as we learned last night.
A family member was sent here at 8pm last night. Multiple cancers through the body and a patient in some of the worst pain a person can be in. By 5 am in the morning my family member had to call 911 and get a ride back to the hospital.
When my family member arrived so late at night, they were not approached by any staff member for procedural check in. Medications needed to be administered every 3 hours and when he ask the nurse they responded with ok and left the room and never returned.
The patient suffered all night in a room the was so cold that they and the other 2 patients in the same room were in agreement. The website rooms do not match what we searched. I am appalled

The food was not good they did not Accommodate with my wife's allergies but the physical therapy ppl where really nice to her and Katrina from Admission was really nice and very helpful and always answered my wife's questions and social services was really helpful with her and Damian the kitchen manager was helpful with my wife's food if there was a problem

Country Hills is awesome! Such a great place for the residents and staff. I've worked closely with the Social Services team, Molly, Mary, Samantha, Belinda, and Eric. They all go above and beyond to take care of the resident's needs, it's wonderful to see! The nursing care is top notch!
Spencer the Administrator has been very helpful too, he really cares about the staff and residents. The team is fantastic!
